Iam Dr Binod Dangal specialized in MD General Practice (MDGP). I am from Sindhupalchok, one of the remote districts of Nepal. The early death of my father hardened my determination to enter into the medical field. Due to excellence in education in village school, I got financial support from an Italian lady, Teresa Niccoli Hassabis, for my schooling and higher education. I was awarded a provincial scholarship for MBBS (Bachelor in Medicine and Bachelor in Surgery) in China and received a Gold Medal in post graduate degree in General Practice in Tribhuvan University Teaching Hospital due to my excellent performance in education. I am also selected as a United Nations Examining Physician (UNEP) for Dolakha Nepal in 2018.
Knowing the unavailability of accessible health care in remote Nepal, I envisioned the concept of UHC (Universal Health Coverage) and started working in Gaurihankar General Hospital as a hospital director as my first journey to save lives in underserved communities of Nepal. To strengthen my skills, I was trained in Ultrasound, Echo and Endoscopy in India and I received scholarship for Global Health Course in Harvard University, USA and as a scholar I did trauma course in David Nott Foundation, London, UK. I have participated in virtual training of "Leadership and Management in Health '' organized by University of Washington, USA.
While working in remote Nepal, I got opportunities for some specific government clinical trainings like HIV/AIDS management, Non Scalpel Vasectomy (NSV), Safe abortion service (SAS), Tuberculosis (TB), Integrated Management of Childhood Illness (IMCI), Sexually Transmitted Infections (STI), Mental Health, COVID-19 management, Burn care and Ear Nose Throat (ENT) care.
I served for more than 10 years in Dolakha and performed thousands of different kinds of surgeries involving orthopedics, gynecology, obstetrics, pediatrics and general surgical specialties. During and after the earthquake of 2015, I served for thousands of earthquake victims in the district performing complicated surgeries inside the tent.
I Am working as an MDGP consultant in Charikot Hospital. This is a very unique hospitals among other public institutions. It serves for more than 80,000 patients every year. This hospital has functional electronic health record (EHR) system for data management, it provides free health care including registration, investigations, treatment and surgeries. Due to resilient effort in healthcare in the district, this hospital has high patient volume similar to tertiary care centers and 20% of people from other districts also visit to take service from this hospital. During COVID pandemic, this hospital introduced PCR testing facility and served as a COVID dedicated hospital in the district.
As a consultant, I am mentoring MD residents coming from Bir Hospital and Kathmandu Medical College. For mid-level practitioners (Nurses and paramedics), I have developed a curriculum based clinical hand book which provides a learning platform through regular continuing medical education (CME). I have been involved in many research activities in the hospital. I have introduced performance based systems in institutional human resource management. I am also leading the collaborative care model of WHO in mental health care.
I have been involved in uplifting rural health care through collaboration and coordination with the support of Health Partnership Nepal (HPN) - a UK based charity organization since 2015, where they have brought a pediatric hernia surgery project to rural Nepal. I am directly supporting senior citizens of my home district as a founder of the senior citizen home to protect vulnerable elderly people. And, I am supporting a few orphans/ vulnerable children for their study.
Over a decade, with continuous hard work in health care, there has been significant reduction in maternal mortality, neonatal mortality (21/100-8/1000 live births) and significant increase in institutional birth rate (57%-85%) and the concept of UHC is met in the district. With this effort, I am honored in the district by the municipality, by the district coordination committee and by the hospital as the best health care provider.
